My Husband Tried Harvesting My Kidney for His Mistress Chapter 1

My phone rang at 6:52 a.m. I was already in the hospital, sitting in the surgeons' lounge with a cup of coffee I hadn't touched, because I'd known this call was coming for three weeks. I just hadn't known it would come this early.

"Winona." Cynthia's voice cracked on the second syllable, right on cue. "Your sister is bleeding out. You're the only match. You need to get here right now."

I looked at the clock. 6:53. "I'm already here," I said, and hung up.

Cynthia Olson has been my stepmother for fifteen years. She has soft hands and a softer voice, and she can cry without her mascara running, which is a skill I used to admire before I understood why she'd needed to practice it. She married my father a year after my mother died. I was eight. I called her Mom by the time I was nine, because she asked me to, gently, the way she asked for everything.

I walked toward the surgical ward with my badge already scanned, my white coat already on, and I felt the corridor tilt into a kind of theater before I even reached it. A dozen people stood outside Nataly's room — nurses who had no business being there, a janitor who'd stopped mid-push with his cart, a woman in street clothes holding her phone up at chest height, the little red dot blinking. Recording.

Cason found me before I found the door.

He fell into step beside me like he'd been waiting around the corner, which he probably had. My husband has a face made for reassurance — square jaw, kind eyes, the kind of face that photographs well next to hospital beds. His hand landed on my shoulder, warm and heavy.

"Let's just get through this together," he said, low, close to my ear. "She's family. Don't make this ugly."

I looked at the phone across the hall. Still recording. I looked at Cason's hand on my shoulder, positioned exactly where a camera would catch it best.

"Nothing about this is ugly yet," I said.

Inside the room, Nataly was propped up on the gurney, IV lines taped to the back of her hand, her face arranged into something between agony and hope. She's my stepsister, three years younger, the kind of pretty that makes strangers want to help her carry things. She caught my eye and let out a small, wet sob, right when the door opened wide enough for the hallway to see her.

"I already had blood drawn this morning," I said, to the room, to the phone, to all of it. "I took an anticoagulant at five a.m. for a procedure I'm consulting on later today. My blood isn't usable right now. Even if I were a match, which hasn't been confirmed by an actual lab report."

Silence. Cason's eyes did something small and fast — a flicker, there and gone, like a man recalculating. Cynthia's hands, hovering near Nataly's blanket, went still. And Nataly's sob rose half an octave, sharper now, less rehearsed.

Cason's hand tightened on my arm. "Can we talk. Just us."

He pulled me into the alcove by the supply closet, out of camera range, which told me everything about what he actually wanted witnessed.

"Don't make this about us," he murmured, his voice gone soft and wounded, the tone he uses when he wants me to feel like the unreasonable one. "Think about what people will say. I'm doing this for us, Winona."

I snapped my surgical gloves tighter, one hand and then the other, an old habit that steadies me before I cut into something. "For us," I repeated.

"Yes."

"Interesting math," I said, and walked back into the hallway.

Cynthia was waiting there, and this time she came for me directly, both hands closing around my wrists, tears already tracking down her face with that same practiced precision. Her rings pressed cold against my skin.

"After everything I've done for you," she said, voice trembling just enough. "Fifteen years, Winona. I raised you like my own. Your mother would have wanted this."

There it was. The blade she always keeps sheathed until she needs it — my mother, dead since I was eight, resurrected as a guilt trip.

I looked down at her hands on my wrists. Soft hands. Expensive rings. The same hands, I was beginning to understand, that had signed off on a check fifteen years ago.

I didn't raise my voice. I let it go soft instead, the way I do right before I tell a patient's family the worst news in the calmest possible tone.

"I know exactly which mother you mean," I said.

Cynthia's face didn't move. But behind her eyes, for just a second, something shifted — not grief. Calculation. And I filed that away too, the way I file everything, in the small leather notebook in my coat pocket, dated, timed, waiting.
